Patient Information

Preparing for Your First Appointment

Before your appointment, please complete the intake and consent forms emailed to you through our online booking platform, JaneApp. To verify if we accept your Extend Health coverage, feel free to call our office beforehand. We also recommend compiling a list of questions to discuss with your therapist.

Your First Visit

Upon arrival, please register at the front desk. We will confirm your insurance information and review necessary paperwork, such as intake forms, referrals, medication lists, and imaging reports.

Your initial appointment will involve a comprehensive discussion of your medical history, previous treatments, current limitations, and other relevant information.

What to bring:

  • A valid photo ID
  • Insurance information, including your insurance card and any necessary authorization or referral forms.
  • A list of current medications, allergies, and any relevant past surgeries.
When To Arrive

Plan to arrive 5-10 minutes before your scheduled appointment to complete any necessary paperwork and settle in. This will also give you time to ask questions and address concerns before your session begins.

What To Wear
  • Comfortable, loose-fitting clothing that allows easy movement and allows the therapist to examine and treat the affected area. For example, if you have a knee issue, consider wearing shorts and for a shoulder issue a tank top is ideal.
  • By way of footwear, we request no outdoor shoes in the facility. You can bring a change of indoor shoes such as sneakers, or you can attend your appointment in socks or bare feet. We do have shoe covers available if you prefer to wear your outdoor shoes.
What To Expect

During your first session, your therapist will perform a comprehensive evaluation that includes a review of your medical history, a discussion on your goals, an assessment of your current condition, and an examination of your range of motion, strength, and movement patterns.

Based on your evaluation findings and goals, our therapist will devise a customized treatment plan for you. This plan will include follow-up appointment recommendations on how long and frequently you should attend therapy.

On the first day, you will likely receive an explanation, education, along with some treatments to alleviate any pain you are experiencing and initiate your recovery. You may also be given home exercises to help support your progress in therapy.

Each session after that will build on the initial evaluation to create the most significant impact possible for each therapy session. The goal will be to help you recover quickly and achieve meaningful, lasting results.

Communicating openly and honestly with our therapist throughout the session is essential, as is sharing any concerns or questions you may have. This will help ensure the most effective treatment and a positive experience. We genuinely want to listen and understand your problem(s) and provide you with the best possible solution!

Our Calgary, AB therapists will keep a detailed record of their findings and treatment plan and can give those to any medical professional of your choosing.

You will arrange the remaining sessions at the front desk before leaving your initial appointment. You may contact us at any time if you have any questions.

Insurance Info

MOVE TO MOVE accepts a wide range of health insurance policies. We can direct-bill your Extended Health Benefit plan using Telus Health Portal, Alberta Blue Cross, Sunlife, Medavie, and Provider Connect which cover a majority of benefit plans in our market.

We also accept and treat patients who have been involved in a MVA (motor vehicle accidents. Please book in with a Physiotherapist or Chiropractor for an initial assessment.

We also have an active contract to provide physiotherapy services for WCB (workers compensation board) claims.

We currently do not have a contract with Alberta Health Services for their Allied Health Services Programs for post-operative, fracture and low-income or seniors.
